The Center for Health Initiatives & Community Engagement is where GRAAHI meets the community — listening, educating, and taking action to improve health outcomes for African Americans across Greater Grand Rapids. We create programs that increase access to care, strengthen health literacy, and champion wellness from every angle: chronic disease prevention, mental and physical well-being, and gender-specific health needs.

Our Programs

Be empowered is a series that aims to empower community members with the tools to navigate the healthcare system with confidence. With a focus on patient self-advocacy and rights, this program uses interactive and collaborative learning, encouraging members to be their own best advocate.

Our next cohort starts April 13th – May 5th!

As part of this program, you will:

  • Hear from others in your community and share health journeys
  • Reduce the prevalence of chronic diseases in the community through new tools

Chronic Disease Prevention & Intervention

GRAAHI’s chronic illness program empowers community members with the knowledge and tools to prevent and manage conditions like kidney disease, diabetes, heart disease and so much more.  Through partnership with local health organizations and congregations, we deliver accessible 12-week cohort-based classes that strengthen health literacy and support long-term wellness in Grand Rapids.

This program empowers our senior community to prioritize their well-being by fostering community support and providing tools to navigate chronic health challenges. In our effort to promote resilience and holistic wellness, we invite seniors and their families to redefine aging and reclaim vibrant living together.

GRAAHI’s Sickle Cell education and Family Support program strengthens health literacy, emotional well-being and access to resources for individuals living with Sickle Cell Disease and their families. Through quarterly educational sessions, specialized workshops, and family-focused support groups, the program aims to:

  • Improve participants’ understanding of Sickle Cell Disease
  • Empower patients and families with strategies to manage symptoms, mental health, and nutrition
  • Build a strong, supportive community network
  • Equip caregivers with self-care and advocacy skills
  • Develop sustainable, peer-led support networks

Be Empowered

This four-week education series aims to empower community members with the knowledge and tools to navigate the healthcare system with autonomy and confidence. With a specific focus on patient self-advocacy and rights, this program utilizes interactive and collaborative learning, encouraging participants to be their own best advocate.

The goal of our PCP Connections Initiative program is to bridge the gap in healthcare access by connecting African American community members, particularly those without a primary care physician (PCP), to healthcare providers. This initiative addresses both systemic healthcare barriers and the mistrust of the healthcare system by providing consistent access to quality care, fostering long-term relationships with healthcare providers, and promoting preventive care.

This quarterly certification program equips community members with essential, hands-on life-saving skills through accessible, partnership-driven training. The program will be held quarterly, providing attendees with CPR and First Aid certification valid for two years. This program strengthens community readiness to respond confidently in emergency situations.

Fit Jam

This communal fitness program blends mindful movement with the arts to create an engaging, family-centered space that supports both physical and mental wellness. Our mission is to offer holistic, culturally rooted health initiatives, this program incorporates partnership with mental health professionals to ensure accessible, supportive wellness experiences for all.

This program aims to create an inclusive space where participants can work with licensed health professionals to improve physical and mental well-being. The 8-week walking and fitness program is designed to engage at-risk adults, showcasing walking as a key form of exercise. While also providing hands-on learning opportunities to enhance physical health literacy, manage chronic conditions, and promote healthier lifestyles.

This new program is designed to empower women in cultivating a healthy and whole lifestyle. FlourishHer takes a comprehensive approach to wellness, addressing key topics such as mental health, breast and cervical cancer awareness, heart disease prevention, and stress and anger management. More than just education, it fosters a supportive, community-focused environment where women can embrace proactive health practices while deepening their connection to themselves and others.

A quarterly barbershop talk series designed to create a safe, intimate environment where African American men can discuss health and wellness, access vital resources, and build a strong support network. Each session features African American physicians, mental health clinicians, and community leaders who provide insight, education, and encouragement.
